State board registers, FDA warning letters, the 503B list and openFDA recalls are searchable by company name — so a provider that will not name its pharmacy has ended the conversation.

Every independent check available to a buyer is searchable by pharmacy name. That makes the name the gate: without it, four of the five public checks cannot be run at all.

Two phrases worth recognising while you ask. "FDA-approved pharmacy" describes something that does not exist — the FDA approves drugs, not pharmacies, and does not approve compounded preparations from any facility. "Pharmaceutical grade" is not a regulatory category.
